Casual RN, 1-2 days a week, some weekends, and occasional on call.

Manages overall care of assigned patients by coordinating and providing nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care. Supports the Provider Relations activities of the organization.

Essential Job Functions

  • Initiates communication with attending physicians, other staffmembers and other agencies as needed to coordinate optimal care and useof resources for assigned patients/families. Maintains regularcommunication with the director of clinical services or patient carecoordinator and physcian to review patient care.

  • Receivespatient assignments from the director of clinical services or patientcare coordinator. Assumes overall responsibility for assignedpatients/families, including assessing, planning, implementing,re-evaluating and revising care as necessary. Obtains data on physical,psychological, social and spiritual factors that may influencepatient/family health status and incorporates that data into the plan ofcare.

  • Maintains up-to-date patient records so that problems,plans, actions and goals are accurately and clearly stated and changesare reflected as they occur.Initiates and sustains, as needed, theimplementation of orders for medications and treatments as prescribed bythe physician in the physician's plan of care.

  • Acceptsresponsibility for managing overall care of the patient by providingdirect patient care and by coordinating the physical care of the patientby teaching primary caregivers, volunteers and employed caregivers asappropriate.

  • Instructs the patient and caregiver how toadminister medication and recognize side effects; how to performpersonal care and oral hygiene; how to assist in lifting, moving,ambulation and exercise; how to prepare for the events preceding death;proper diet and other instructions as ordered by physician.

  • Informsthe director of clinical services or patient care coordinator ofunusual or potentially problematic patient/family issues. Providesappropriate support at time of death. Shares in providing 24-hour,seven-day-a-week coverage to patients and families.

  • Makes home health aide assignments, prepares written instructions for the aide and supervises the aide in the home.

  • Demonstrates familiarity with policies of the company and state and federal regulations pertaining to nursing services.

  • Participateson committees and special projects and other related duties asassigned. Contributes to team effort by accomplishing related resultsas needed. Accepts responsibility of assignment to perform specializedprocedures, such as IV therapy, only when qualified with specialtytraining, proven competency, clinical background and expertise in thespecialty area. From time to time this position will be expected toperform on-call professional duties.

  • Personnel are expected toparticipate in appropriate continuing education as may be requestedand/or required by their immediate supervisor. In addition, personnelare expected to accept personal responsibility for other educationalactivities to enhance job-related skills and abilities. All personnelmust attend mandatory educational programs.

Qualifications

  • Must be a registered nurse with a current state license.

  • Must have current certification for CPR.

  • Must have a minimum of 1-year general nursing experience with current Hospice, medical, surgical or critical care experience. Experience with providing pain management and IV skills preferred.

  • Must demonstrate a commitment to the company's philosophy of care.

  • Must have experience in providing services to terminally ill patients.

  • Must be a licensed driver with a reliable automobile that is insured in accordance with state and/or organization requirements.

  • Must meet all local health regulations, pass post-offer drug test and pass post-employment physical exam if required.

  • Must be capable of maintaining regular attendance
