Hours of Work: Forty (40) plus. 8:00 a.m. - 4:30 p.m. (scheduled services shall include evening and weekend hours and to be accessible to the consumer (24) hours a day (7) days per week in time of crisis, etc. EDUCATION, TRAINING, AND EXPERIENCE:1. Bachelor's degree required or may consider post high school course work (Associate Degree is the minimum required education) in psychology, social work, sociology and/or other related fields, with specific training relative to chemical dependency, or mental health and recovery. 2 Two years' experience in the delivery of mental illness/substance abuse services or other related human service programs. Demonstrated skills and competencies to work with people with mental illness/substance abuse. 3. Must be authorized for coverage under agency insurance if operating agency vehicle. Must have good driving record, valid Nebraska driver's license, and/or required level of personal auto insurance. GENERAL DESCRIPTION: Community Support is a community-based service designed to help ensure participants (eligible persons disabled by severe and persistent mental illness and/or whose primary diagnosis is substance dependency) develop and utilize a recovery plan and connect within their community for resources towards stabilizing in their communities and reduce hospitalizations. Goodwill has a contractual arrangement with Region 3 and the Department of Health and Human Services: Division of Behavioral Health. Develop the treatment plan: Coordinate completion of initial and annual strength-based assessments to identify each participant's recovery vision. Utilize the assessments to facilitate the development of an (ITTP) Integrated Treatment Team Plan. Update/revise the plan's goals, objectives, interventions on a 90 day routine basis, or as needed.2. Facilitate and track progress of the treatment plan: Provide interventions in a trauma informed manner to create successful outcomes with the participant towards recovery goal achievement.3. Develop and coordinate a treatment team: Assist the participant in reaching out to develop a treatment team comprised of both natural and professional supports. Foster a positive, supportive relationship with the participant and their team members involved in the treatment team, including both natural and professional supports. Act as a co-chair, with the participant.4. Provide crisis management: Support and assist the participant in times of stress and crisis, in coordination with other team members. Utilize a Wellness Crisis Support Plan developed by the participant. Follow agency reporting policies and procedures.5. Community resource acquisition: Link participants with needed services within their community as identified in the treatment plan and/or to meet basic/crisis needs that arise.6. Adhere to program documentation policies and procedures: Track data or client demographics, outcomes, service providers, referrals, goal progress, level of engagement in recovery, and related information, etc. in the participant's electronic file. Complete documentation in a timely manner as required, including the development of the ITTP, goal review process, and progress notes. Competently present each case's medical necessity for authorization and reauthorization to Managed Care entities to ensure the service provided will be a billable service.7. Provide face-to-face community-based services: Provide and document required face-to-face community-based contacts on a monthly basis, per contract specification (Medicaid and Region requirements) and budgeted unit utilization.8. Clinical Supervision: Actively participate in routine (minimum of monthly) Clinical Supervision meetings to review the treatment plan, determine appropriate level of care, and explore
